Candidate must be a graduate of an
ADA-accredited dental school unless accepted into a residency program or fellowship at a
Board-approved tranining facility.

Candidate must have a letter from the
supervising dentist indicating the location of practice and name of the dentist.
Candidate must submit official transcripts
from their dental school.
Candidate must sucessfully complete written
examinations (sterilization/infection control and jurisprudence)
The non-refundable fee is $150.00.
